1. These forms are available online through Google. Upon submitting these forms you do NOT need to send in a paper form with your student. When completing the Google Forms, if payment is required, you will be given the option of sending in cash/check with your student or paying with your credit card at the PTA Store. Please clearly label the envelopes with money or checks sent in for the "PTA".

2. When using the paper forms, please send check or cash, if payment is required, in with your student. You do NOT need to print and send in hard copy forms if you are paying for purchases through our PTA store.
